Positional Release

Positional Release Technique (PRT) is a gentle, passive manual therapy method used by massage therapists to relieve pain, muscle tension, and dysfunction by positioning the body or its tissues in a state of greatest comfort, thereby reducing neuromuscular activity at specific tender or trigger points. Unlike traditional stretching or direct compression, PRT involves moving the affected area away from pain and maintaining this comfortable position—often for 60 to 90 seconds—until a neuromuscular release occurs.

Key Features

  • The technique is based on the principle that placing tissues in a position of maximal ease helps “reset” abnormal feedback loops at the muscle spindle level, decreasing spasm and pain.

  • The therapist identifies a tender or dysfunctional area, moves the body or joint to a position where tenderness is minimized, and holds that position until muscular tension and discomfort are significantly reduced (often by at least 70%).

  • Types of PRT include Strain Counterstrain, Functional Technique (following tissue ease directions), and methods incorporating props or fascial stacking.

Purposes and Benefits

  • PRT interrupts pain-spasm cycles, releases trigger points, and relaxes tissues that may not respond well to typical massage techniques or direct pressure.

  • The technique is effective for both acute and chronic conditions, including muscle spasms, joint pain, postural issues, and sports injuries.

  • It improves circulation, reduces muscle guarding, lengthens tissues, and can provide immediate relief without the discomfort associated with more aggressive techniques.

Typical Use in Massage Sessions

  • Therapists incorporate PRT when direct pressure is intolerable or ineffective, or to speed up relief at the start of a session before progressing to deeper work.

  • It is safe for most populations and is particularly suited to clients with high pain sensitivity, acute injury, or chronic pain syndromes.

  • The technique integrates well with myofascial release, trigger point therapy, and other neuromuscular approaches.

In summary, Positional Release Technique is a gentle, effective strategy that helps massage therapists resolve pain and tissue dysfunction by moving tissues into positions of ease and triggering a reflexive release in the neuromuscular system.
